VCU Health Tappahannock Hospital has received the highest possible ranking of ‘A’ from The Leapfrog Group’s Fall 2023 Leapfrog Hospital Safety Grade, marking its sixth achievement of this laudable recognition. The Leapfrog Group, a nationally recognized non-profit organization dedicated to the enhancement of healthcare quality and safety, issues these safety grades biannually to participating hospitals nationwide.
This grading program is unique in that it strictly evaluates hospitals on their proactive efforts to prevent medical errors, accidents, injuries, infections, and other detrimental impacts on patient health and wellbeing. Each grade is a testament to a hospital’s commitment to creating a safe patient environment. Liz Martin, the president of VCU Health Tappahannock Hospital, attested to the collective responsibility within the organization to ensure patient safety. She emphasized that their consistent attention to safety is a fundamental pillar within their commitment to the community.
The grading system considers various clinical aspects, including medication safety, intensive care unit (ICU) staffing, adherence to patient safety best practices, and nurse staffing evaluations. Amongst those factors, one highlights Tappahannock Hospital’s excellent outcomes such as low rates of hospital events that harm patients, effective procedures for recognizing and reducing potential safety problems, well-established infection control protocols, and an effective staffing plan to ensure manageable workloads for nurses and sustained ICU physician coverage. The hospital has sustained more than 95% compliance across all units in the administration of medications through a barcoding process, which verifies the correct patient, medication, dosage, administration time, and potential interactions. This system contributes significantly to patient safety. Additionally, the utilization of technology by team members to provide specialized physician teleconsultations, assisted by daily coverage from the VCU Medical Center critical care team, further enhances patient care. The hospital culture prioritizes safety, with assistance from the senior management engaging the front-line personnel to recognize potential safety incidents and facilitate process improvements to prevent harm.
The Leapfrog Hospital Safety Grade is designed under the guidance of a national expert panel, the system uses up to 27 measures of publicly accessible hospital safety data to assign grades to almost 3,000 general hospitals across the nation. The grading methodology is peer-reviewed, fully transparent, and readily available to the public.
