A hospital privileging patient safety and achieving high-performance measures across these categories is rewarded with a star rating, with a maximum of five stars confirming the highest quality of healthcare services.

Contrary to public perception, the CMS 2025 ratings have been quite favorable for VA hospitals. A massive 77% of VA hospitals that were assessed clinched an Overall Hospital Quality Star Rating of four or five stars, while none had fallen to a one-star rating. Looking at the performance trajectory, more than 90% of VA hospitals with ratings either maintained or improved their 2024-star rating, further underscoring their commitment to continuous quality improvement.
