Strategic Collaboration: How Saudi Arabia Reduced CLABSI Rates through a Comprehensive Public Health Initiative

The year 2021 brought a considerable public health predicament for Saudi Arabia as it grappled with a startling prevalence of Central line-associated bloodstream infections (CLABSI) in adult Intensive Care Units (ICUs). Fast forward to the present, the Kingdom has successfully navigated this crisis, achieving a 48.8% reduction in CLABSI incidents through the implementation of a comprehensive initiative.

Led by the Ministry of Health, the initiative was designed to tackle the alarming CLABSI rates that stemmed from improper handling of central catheters, which when incorrectly inserted, maintained, or removed, introduced germs into patients’ bloodstreams. This adverse event was devastating for patients, particularly those already struggling against an illness and thus more vulnerable to infections. Not only did CLABSI cause patient harm and distress, but it also exacerbated healthcare costs and extended hospitalization periods.

Realizing the urgency of the situation, the Ministry of Health launched a nationwide initiative in 2022 to combat CLABSI. In collaboration with entities such as the General Directorate of Infection Prevention and Control (GDIPC), Public Health Authority (Weqaya), and the Ministry of Defence Health Services, the campaign targeted all hospitals containing adult medical and surgical ICUs. A task force, including representation from the Ministry of Health, regional health authorities, IPC professionals, and healthcare providers, was formed to lead the charge.

Initial evaluations revealed areas in dire need of immediate intervention. Further scrutiny revealed two principal factors contributing to the high CLABSI incidence rate: lack of awareness and substandard practices concerning the handling of central catheters. A strategic plan, incorporating these findings, was then developed and set into motion.

The initiative’s results were staggering. CLABSI rates plummeted from 2.5 per 1000 days in 2021 to 1.28 per 1000 days by the third quarter of 2024. The initiative also indirectly impacted other healthcare-associated infection rates, which likewise showed declines. A total of 222 healthcare facilities have reaped the benefits of the initiative thus far, with approximately 5000 healthcare providers undergoing training, leading to a 98% adherence rate to the central-line maintenance bundle by 2024.

The significant decrease in the prevalence of CLABSI incidence in Saudi Arabia is emblematic of the power of strategic planning, reciprocal collaboration, and implementation of best practices. The cohesive effort between governmental authorities, stakeholders, clinical practitioners, and data-led decision making contributed immensely towards the success of the initiative.

While celebrating this initial triumph, the initiative continues unabatedly, maintaining rigorous training and competency assessments to uphold high-standard IPC practices. Along with this, regular updates to the surveillance system ensure the accuracy of data collection and analysis while identifying potential areas of improvement through feedback collection from healthcare providers.

Looking into the future, efforts to expand the initiative to more healthcare facilities are planned, with consideration given to the integration of advanced technologies to boost surveillance capabilities and data analysis processes. The General Directorate of Infection Prevention and Control, under the aegis of the Ministry of Health, leads this ongoing project. More health and infection prevention control is of paramount importance for the country’s healthcare sector.
