Rising Concerns: The Escalation of Carbapenemase-Producing Infections and Antibiotic Misuse

A recent study published in Antimicrobial Resistance and Infection Control underscores a concerning trend in Canadian acute-care hospitals related to the escalating incidence of carbapenemase-producing Enterobacterales (CPE) infection and colonization. The study utilized surveillance data collected through the Canadian Nosocomial Infection Surveillance Program (CNISP) and revealed that though the incidences were initially low, there is now an exponential rise. It revealed an increase in the CPE infection incidence rate from 0.03 per 10,000 patient-days in 2010 to 0.14 in 2023.

CPE are bacterial pathogens exhibiting resistance to multiple antibiotic classes which pose serious treatment challenges. Notably, an earlier analysis of CNISP data from 2010 to 2014 showed no significant increase in the incidence of CPE, which is in stark contrast to more recent observations. Factors such as patient’s age, existing comorbidities, international travel or receipt of medical care abroad have been linked with CPE infection and colonization.

The study also raised alarming concerns about the prevalence of healthcare-associated (HA) CPE infections noting an increased incidence from 2019 through 2023. A majority of these infections were reported from seven hospitals that accounted for over half of all HA-CPE infections in 2023.

On the international front, the World Health Organization (WHO) reported new cases of H5N1 avian flu in Bangladesh and India. The report emphasised the importance of maintaining high levels of surveillance and introducing improved control measures to curtail the spread of infections.

Concurrently, another investigation conducted at the Department of Veterans Affairs (VA) outpatient clinics highlighted an urgent requirement for better diagnostic stewardship. The research found prevalent antibiotic prescribing for asymptomatic bacteriuria (ASB), potentially leading to unnecessary antibiotic exposure and an elevated risk of antimicrobial resistance.

In conclusion, these studies highlight significant emerging challenges in infection control and prevention. They bring into sharp focus the mounting problems of growing bacterial resistance and misuse of antibiotics, calling for improved infection control measures, enhanced antimicrobial stewardship, and access to newer antimicrobials.
