The Leapfrog Group, renowned for its commitment to driving forward patient safety, has unveiled its Fall 2025 Hospital Safety Grades. Acting as a biannual evaluation, these grades provide a comprehensive look at the capacity of hospitals to safeguard their patients from preventable infections, harm, and medical errors. Distinctly, the grades examine performance across an impressive 31 safety parameters, encapsulating infection control, medication safety, and the effectiveness of staff communication among others. These assessment criteria present a critical picture of the current state of the healthcare industry in terms of patient safety.

In the Rockford area, the report illuminates both strides made in the right direction as well as existing apprehensions. Peculiarly, Mercyhealth – Riverside fell off the Leapfrog’s list in their latest evaluation. A noteworthy non-participant was the Javon Bea Hospital – Riverside, the reasons for their non-compliance is left unanswered by the Leapfrog report. As a country-wide initiative, thousands of healthcare facilities across America actively participate in the Leapfrog Hospital Survey to furnish patients with data to guide their healthcare decisions – yet some, like the aforementioned, do not see fit to engage. This avoidance indeed raises suspicions regarding their reluctance.

Distinctly, the report represents a double-edged sword for the Rockford community, delineating continuous progress at facilities such as OSF, while simultaneously casting light on emerging challenges being faced at SwedishAmerican. The Northwestern Medicine Kishwaukee Hospital, located about 40 miles to the east, has once again garnered an ‘A’ grade – marking its fifth consecutive achievement. Leapfrog attributed the hospital’s measurable success to its impressive infection prevention and staffing protocols, two primary areas many other facilities grapple with.

In terms of overall hospital safety, Illinois earned the 20th spot nationwide. A promising thirty-five hospitals in the state earned an ‘A’, underscoring consistent improvement albeit with a spectrum of uneven results across the state. As a longstanding resident of Rockford, while it is heartening to witness continued performance at OSF, the Leapfrog grades serve as a poignant reminder that hospital safety is an ongoing commitment.
