Mission Hospital, situated in Asheville, North Carolina has recently stumbled into an unstable health care situation that screams for urgent attention, according to the North Carolina Department of Health and Human Services (NCDHHS). The hospital has been cast into a state of ‘Immediate Jeopardy’. For a clearer comprehension, the context of ‘Immediate Jeopardy’ refers to circumstances where noncompliance within a hospital has heightened the potential of causing serious harm, severe impairment or even death among patients under its care.
This health crisis was unleveled post a comprehensive review by the Centers for Medicare and Medicaid Services (CMS). The review, encapsulated under a ‘Summary Statement of Deficiencies’, shed light on severe malpractices breaching hospital conditions of participation. Stories of neglect and misjudgment from the document act as stark reminders about the paramount importance of compliance with healthcare standards and protocols.
In one of the unveiled instances, a medical team overlooked notifying the consulting physician about an 88-year-old patient’s dwindling hemoglobin levels and dropping oxygen saturation prior to her death. Further, the team’s negligence stretched to the point of skipping the monitoring of the patient’s vital signs for more than half a day.
In a separate case, a high-risk medication, Droperidol, carrying an FDA black box warning, was administered to a 55-year-old patient undergoing Fentanyl withdrawal. Resultantly, the patient fell into cardiac arrest in a mere half-hour post medication and tragically succumbed.
Another notable case concerned a pair of seven-year-old twins who, despite presenting the distinctive ‘three C’s’ of measles recognition- cough, coryza, and conjunctivitis- along with a fever and rash, were left in an open waiting room for 30 minutes. This was followed by transferring them to a crowded patient care area without implementing the necessary isolation precautions. The hospital’s procedural failures led to measles exposure for 26 individuals. Despite the staff’s prior training to recognize measles symptoms, the lack of immediate identification and isolation exposed everyone in the hospital to a significant risk for serious infectious disease transmission.
In light of these failings in 2025 and 2026, the incidents culminated in an Immediate Jeopardy investigation commencing in January 2026. A commitment towards amending repeated quality concerns and system faults was voiced by Memorial Mission Hospital. They set forth a plan to rectify the deficiencies discovered during the survey, satisfying the conditions for CMS’s approval.
