Exploring Antimicrobial Use in Hospital-at-Home Settings Across Mayo Clinic Sites: A Call for Further Stewardship

A comprehensive evaluation of antimicrobial use among patients in the hospital-at-home (HaH) programs across three Mayo Clinic sites was conducted from the years 2023 to 2024. The findings of the investigation revealed that a significant majority of the patients, nearly three-quarters, were administered an antibiotic during their HaH admission period. The most frequently used antibiotic was found to be Ceftriaxone, a cephalosporin antibiotic that treats bacterial infections, and the top antimicrobial indication was tied to respiratory infections.

Given these revealing statistics and the current urgency in promoting prudent use of antibiotics, further investigation and strategic intervention into potential stewardship opportunities within the HaH sector is deemed essential. This study clearly underscores the need for more rigorous antibiotic stewardship within the sector, as it can significantly contribute to overarching goals of infection prevention. The promotion of targeted antibiotic stewardship initiatives can help improve patient outcomes, reduce antibiotic resistance, and ultimately lead towards a more sustainable healthcare approach.

While the study findings have shed light on the general patterns of antimicrobial use in HaH settings, a closer examination of the underlying factors contributing to these trends is necessary to propose effective solutions and interventions. The collective contribution of healthcare professionals, patients, and policy-makers is fundamental to realizing this objective. More extensive research, context-specific interventions, and committed alliance are the key pillars to bolster responsible antimicrobial use and stewardship in the healthcare sector.
