Abrupt Termination of Global Health Programs SMART4TB and Project ACCELERATE: Consequences and a Call to Action

It is with great sorrow that we announce the premature conclusion of two pivotal global health initiatives, SMART4TB and Project ACCELERATE, casualties of federal funding cuts. These initiatives have significantly equipped us to combat tuberculosis (TB) and HIV/AIDS—two of our deadliest infectious foes. Their sudden absence will globally reverberate in terms of the millions they served, the unfinished research they left behind, and the progress they were compelled to abort.

Johns Hopkins University, the nurturer of these initiatives, announced their termination recently. These programs, the bright torches lighting the path of global health and infectious disease control and prevention, were born of an unyielding drive for scientific progression and human service. Their function was noble, their influence irrefutable, and their legacy indelible – having saved lives, engendered knowledge, and fostered hope.

The unfortunate decision to terminate these programs is not merely a repercussion of the present US administration, but a disaster with domino effects rippling through communities, healthcare establishments, and research facilities worldwide.

SMART4TB, devoted to the eradication of tuberculosis, was established in 2022 with a lofty ambition of revolutionizing tuberculosis research, treatment, and prevention. It aligned an international cadre of researchers, clinicians, and advocates with a shared mission of altering TB’s trajectory. However, owing to a political verdict, SMART4TB was abruptly discontinued, with vital clinical trials prematurely terminated, international cooperation severed, and a program with great promise left unfinished and silenced.

Project ACCELERATE, born in 2019, was India’s steadfast spearhead in the fight against HIV/AIDS. It was dedicated to mitigating service gaps for marginalized communities. It revolutionized HIV care with innovative, community-centric solutions, transforming care models, establishing comprehensive transgender health clinics, and broadening HIV prevention amongst vulnerable youth. Nonetheless, Project ACCELERATE too was suddenly put on hold in early 2025 and officially terminated within a month.

This is not just a tale of two lost programs, but a call for global action. A plea to safeguard programs like these that serve as lifelines to millions worldwide needing healthcare access and advancements in global health via research. Unless we safeguard funding for vital research and life-saving interventions, we dig graves for patients relying on these programs.

As we mourn these losses, may these not only reflect the progress lost but also echo a warning and plea to the world: We must guard against the deterioration of global health funding and scientific research to prevent future vacancies being filled with the countless individuals left deprived of healthcare and the promises of scientific innovations and efforts in global healthcare and development.
